For your convenience, Clarity's staff are available to check benefits before your appointment is scheduled. A quote of benefits and/or authorization does not guarantee payment of verify eligibility. Payment of benefits are subject to all terms, conditions, limitations, and exclusions of the member's contract at the time of service.
If your health insurance denies payment, it is the responsibility of the patient to be personally pay their appointment costs in full. Although Clarity is sending documentation to your insurance company, you are responsible for the full amount billed if your insurance does not make a payment. Services may be covered in full or part by your medical and/or mental health insurance company. We advise you to also check your coverage carefully by asking the following questions to your insurance:
Do I have mental health benefits?
What is my co-payment/co-insurance?
Is there a calendar year deductible or does it restart at a different time of year?
How many sessions a year does my health insurance cover?
Does my insurance cover mental health pre-surgery assessments?
At the time of your appointment, you will be charged for the amount Clarity2 was quoted by your insurance company. Whether it be your copay, coinsurance, or deductible. Being a telehealth company, we accept only credit cards as payment. Prior to your appointment, you will be asked for a credit card that must be on file before you can be seen as a patient.
Cancellation/No Show Policy
We have a strict 48-hour cancellation/no-show policy for all appointments. Failure to cancel/reschedule at least 48 hours prior to your appointment OR an absence will result in a $125 penalty fee. Insurance does not cover no show/late cancellation fees. As a courtesy to other patients and their clinicians, your appointment time is reserved exclusively for you and it is difficult to fill your reserved time without proper notice.
